Northwest Pediatrics participates in most managed healthcare insurance plans. Please see the detailed list of plans below for your reference. This list may change from time to time so it is always best to call the member service number on your insurance card to verify that Northwest Pediatrics and our Physicians are members on your specific plan.

Many plans require that you make a co-payment or pay a percentage of the service at the time of your visit. To make this requirement easy on you and convenient, we accept cash, check, Visa, MasterCard and Discover credit cards.

IF YOU ARE ON AN HMO PLAN, PLEASE CALL YOUR MEMBER SERVICES AND MAKE SURE YOU LIST US AS YOUR PRIMARY CARE PHYSICIAN.

NEWBORNS MUST BE ADDED TO YOUR INSURANCE PLAN WITHIN 30 DAYS OF DATE OF BIRTH. YOU SHOULD ALSO ADVISE YOUR INSURANCE PLAN OF THE NAME OF YOUR PHYSICIAN.

Upon arrival to our offices, please present your most recent insurance card to ensure we have the most accurate information, and that your coverage is valid at the time of service. Incorrect or out of date information can result in the patient being responsible for full payment.

We will gladly assist in the resolution of insurance claims. We require that you provide your claim forms and policy information as this is not provided to our practice by the insurance companies.

Payment
It is the requirement of most insurance companies that a co-payment be made at the time of service. Payment may is made with cash, check, MasterCard, Visa and Discover. Your co-payment is determined by your insurance company, and is usually posted on the front of your card. There will be a $20 processing fee for all patients who do not provide co-payment at the time of service.

Self Pays
At the time of service, all self payments are due. After seeing the physician, it is required that you stop at the front desk to determine if there are additional charges.
